Testing & treatment follow-up

A test result is not the end of care.

Clinically appropriate ordering, results review, treatment monitoring, and patient education connected to an accountable plan for care.

How the care is organized

Clinical responsibilities

Appropriate ordering

Testing belongs within an accountable plan for care. The clinical team determines whether an order is appropriate within the program’s scope and the patient’s evaluation.

Results and communication

Results review needs an identified clinical owner and a clear communication workflow. Administrative support can coordinate the process; interpretation and treatment decisions remain clinical responsibilities.

Monitoring and handoffs

Treatment monitoring and patient education continue beyond the initial encounter. Clear documentation and handoffs help the receiving team understand what has been completed and what still needs attention.
