Clear information. Accessible policies.

Read our website terms, privacy policies, and patient information below. Each document opens on this page and can be printed separately. For questions about an existing course of care, use your treating program's secure contact channel.

01Website Terms & ConditionsPermitted website use, service boundaries, and the agreements that govern an engagement.+

Last updated September 8, 2026

1. Scope of these terms

These terms apply to the public LPMD Medical Group website and its informational content. LPMD Medical Group, or LPMDMG, is the shared public-facing medical-group name. LPMD Health LLC, operating as Exilara, provides nonclinical website administration for the group. In these terms, "we," "us," and "our" refer to the website administrator and, only as applicable to their own content and responsibilities, the identified professional entities.

These terms address website use. They do not replace a signed management services agreement, clinical services agreement, employment or contractor agreement, patient financial agreement, informed consent, or an applicable patient privacy notice. A listing on this website is not an offer that can be accepted by submitting an inquiry.

2. Informational purpose; no medical or legal advice

The website describes the group and potential services. It does not diagnose, prescribe, provide individualized medical advice, or establish a clinician-patient relationship. Do not start, stop, or change treatment based on this content. Questions about your health belong with an appropriately licensed clinician.

Descriptions of corporate practice of medicine, licensure, management relationships, and clinical operations are general organizational information, not legal advice or a legal opinion on your business. Obtain advice from qualified counsel about your specific arrangement.

3. Emergencies and current patient care

This website, general email, and partnership contact form are not monitored as clinical-care or emergency channels. For a medical emergency, call 911. Existing patients should use the secure portal or clinical support contact supplied by the program treating them for symptoms, prescriptions, results, follow-up, and medical records.

4. Service availability and professional authority

Services are subject to an agreed scope, appropriate clinical assessment, staffing, entity authorization, clinician licensure, training, competence, and the requirements of the patient's location and care setting. National scope does not mean that every service, clinician, or professional entity is available in every jurisdiction. No prescription, procedure, clinical outcome, launch date, regulatory approval, or business result is guaranteed.

Clinical recommendations, prescribing, and clinical-quality determinations remain with the appropriately authorized clinical professionals. MSO, website, pharmacy, or technology participation does not transfer that authority.

5. Your use of the website

You may browse the site and save or print reasonable portions for personal information or legitimate internal business evaluation, retaining relevant notices. Do not misrepresent your identity or authority; submit another person's confidential information without permission; introduce malware; bypass access controls; interfere with the site; or use the site for unlawful, deceptive, harassing, or infringing activity.

Automated access must respect applicable law, reasonable rate limits, and published technical restrictions. Nothing here prohibits lawful accessibility tools, ordinary search indexing consistent with those restrictions, or legally protected activity.

6. Information you submit

Provide accurate business or professional contact information and only information you have authority to share. Do not submit patient records, Social Security numbers, payment-card details, passwords, sensitive personal information, or confidential business material requiring a separate nondisclosure agreement through general inquiries.

An inquiry does not create confidentiality obligations beyond applicable law and an existing agreement. This provision does not waive privacy duties or authorize unrestricted use of personal information. The Website Privacy Policy explains handling of website-related information. Merely visiting the site is not consent to marketing, the sale of health information, or another activity that requires separate consent.

7. Intellectual property

The website design, original content, logos, and other materials are owned by or licensed to the relevant rights holders. Except for permitted use described above or rights provided by law, do not reproduce, modify, republish, sell, or imply endorsement through the materials without permission. References to third-party names do not imply that those organizations endorse LPMDMG. No clinical license, trademark license, or right to use the medical group's name is granted by this website.

8. External websites and services

Links may lead to independently operated websites or to services administered through a third-party platform, including our external business inquiry form. The destination's terms and privacy practices apply to its platform activities; our own handling of information received through an inquiry remains subject to our applicable notices and responsibilities. A link is not a guarantee of a third party's services or security.

9. Fees and commercial arrangements

The site does not collect payments, enroll patients, or establish a universal fee schedule. Clinical services are 100% cash-pay. Each care program or engagement identifies its fees, inclusions, medication and laboratory charges, cancellation terms, and other applicable financial obligations. Insurance reimbursement is not promised. Legally required patient billing protections remain in effect.

10. Website availability and limitations

We seek to keep website information accurate, but it may be incomplete, change, or become unavailable. To the extent permitted by law, informational website content is provided without a warranty of uninterrupted availability or suitability for a particular business purpose. Confirm material facts with the responsible team before relying on them for a launch or engagement.

To the extent permitted by law, we are not responsible for indirect or consequential losses arising solely from the use or unavailability of this informational website. Nothing in these terms limits liability that cannot lawfully be limited, excuses fraud, gross negligence, or willful misconduct, releases professional duties, waives a patient's claims concerning clinical care, or restricts statutory privacy or consumer rights.

11. Changes, governing law, and separate agreements

We may update these terms and identify the revised date. Material changes will be communicated as required by law; changes do not retroactively remove accrued rights. Applicable United States federal law and relevant state law govern. These website terms do not impose mandatory arbitration, a class-action waiver, or an exclusive forum. A separately executed agreement may address those matters for that engagement. If a provision cannot be enforced, the remaining provisions continue to the extent permitted by law.

12. Contact

For website or administrative questions, email [email protected] with "LPMDMG Website" in the subject, or use the business inquiry form (opens in a new tab). For care-related matters, contact your treating program.

02Website Privacy PolicyInformation associated with this public website and business or professional inquiries.+

Last updated September 8, 2026

1. Who and what this policy covers

This policy covers the public LPMD Medical Group website and information received in connection with its business, professional, accessibility, and privacy inquiries. LPMD Health LLC, operating as Exilara, administers this website on behalf of LPMDMG. The professional entities remain responsible for their own clinical activities and records. The group name does not combine all entities into one legal person or automatically authorize information sharing among them.

This is a website privacy policy, not a HIPAA Notice of Privacy Practices, a treatment consent, or a description of every partner clinic's systems. For medical records and patient-care information, see Patient Privacy & Records and the notice issued by the treating entity. Our Washington Consumer Health Data Privacy Policy separately addresses covered consumer health data within its stated scope.

2. Information you provide

When you contact us, we may receive your name, company, professional role, email address, telephone number, referral source, the content of your message, and subsequent correspondence. A professional inquiry may include qualifications and experience you choose to provide. We use only the information reasonably needed to understand and respond to the request.

The site has no patient intake form, appointment checkout, payment collection, or file-upload feature. Its inquiry buttons link to an externally hosted Zoho form. Please do not submit medical records, symptoms, patient identifiers, government identification numbers, financial account details, or other sensitive information in a general inquiry. A warning against submission does not remove any legal protection applicable to information we receive.

3. Technical information and outside resources

Visiting a website transmits technical information to its hosting and security systems, which may include an IP address, browser and device information, requested page or file, date and time, referring-page information, and security or error events. This information may be used to deliver the page, maintain security, diagnose faults, and respond to abuse.

This site requests typefaces from Google Fonts. Google receives the technical information needed for those requests, such as IP address and browser information. We use a no-referrer policy to limit referral information sent from this page. The logo and other page artwork are included with the site. External contact forms and linked websites are not embedded and load only when you follow their links.

The website application does not include advertising pixels, behavioral analytics, retargeting, session replay, chat tracking, or a social-media tracking widget. It does not request precise geolocation, read patient records, or create a health profile from your browsing. See Cookies & Privacy Choices for the distinction between page code, hosting systems, and linked services.

4. Why we use information

  • Respond to and route a business, professional, accessibility, privacy, or administrative inquiry.
  • Evaluate a proposed partnership or professional opportunity and communicate about it.
  • Operate, secure, troubleshoot, and maintain the public website and related communications.
  • Maintain appropriate records of requests and responses, establish or defend legal rights, and meet legal obligations.

We do not treat a general inquiry as consent to unrelated marketing or a request for medical treatment. Separate notice and consent are obtained where required for a new use. We do not use this website to make automated decisions producing legal or similarly significant effects about visitors.

5. How information may be disclosed

Information may be provided to personnel and service providers who need it to host or secure the site, operate email or the inquiry platform, administer a request, or support an authorized business function. Providers process information for the relevant service and are subject to applicable contractual and legal restrictions.

Where necessary for your request and legally permitted, we may route relevant information to LPMD Health LLC or the appropriate professional entity: LPMD of CA PC, LPMD of NJ PC, LPMD of WY PC, LPMD of NE PC, LPMD of KS PC, and LPMD of TN PC. Aesthetic and Venture are engagement brands, not an automatic permission to circulate your information across every business. A proposed independent MSO partner does not receive your information merely because it works with the group.

Information may also be disclosed to professional advisers, in response to valid legal requirements, to address fraud or security threats, or in a business transaction subject to applicable safeguards and notice requirements. More protective rules governing health information continue to apply. We do not give advertisers access to information through this website or operate a sale, targeted-advertising, or cross-context behavioral-advertising feature on it.

6. Retention and protection

Website-related information is retained only for the period reasonably needed for its purpose, considering the inquiry, an ongoing relationship, necessary follow-up, security investigations, legal obligations, and preservation requirements. Clinical records follow separate retention requirements and are not governed by an ordinary website deletion request.

Access should be limited to the people and systems that need it for an authorized purpose. No website, email system, or internet transmission can be guaranteed secure. Do not send sensitive health information through general email or the business form. If more sensitive information is genuinely needed, request the appropriate secure process first.

7. Privacy requests and available rights

Depending on where you live, the information involved, and the law that applies, you may have rights to confirm processing; access or receive a copy of information; correct inaccuracies; request deletion; withdraw consent; restrict certain sensitive-data uses; or opt out of a legally defined sale, sharing, targeted advertising, or certain profiling. Exemptions, legal retention duties, and verification requirements may apply.

To make a website privacy request, email [email protected] using "LPMDMG Privacy Request" or send a minimal request through the contact form (opens in a new tab). Identify the request, your relationship to the group, your state, and a safe way to respond. Do not attach identification documents or medical records. We may request additional information through an appropriate channel to verify identity or an authorized agent's authority.

We respond within the period required by applicable law and explain an extension or denial where required. Where a right of appeal applies, reply to the decision or use the same contact channel with "Privacy Appeal." We will provide the appeal outcome and available regulator contact information as required. Exercising a privacy right will not result in unlawful discrimination.

8. California and other state rights

Where the California Consumer Privacy Act or another applicable state law covers the relevant information and activity, available rights may include knowing the categories, sources, purposes, and recipients of personal information; access to specific information; correction; deletion; authorized-agent requests; and applicable opt-out or sensitive-information rights. Medical information exemptions are information-specific and do not mean all website or business-contact information is unprotected.

This public website does not operate sale or sharing for cross-context behavioral advertising. A Global Privacy Control signal does not need to disable any such feature here because none is enabled. This statement does not extend to an independently operated website you visit through a link. Requests concerning information outside this page can be submitted using the process above.

9. Children, external services, and international access

This business-facing website is not directed to children under 13 and does not offer them online enrollment. Contact us if you believe a child has submitted personal information through a website inquiry. Any care for minors requires a separate authorized clinical program and its applicable consent and privacy processes.

The site describes United States services. Accessing it from another country does not establish service availability there. Information sent to our United States operations or service providers may be processed in the jurisdictions where those operations and providers work, subject to applicable requirements. Linked platforms publish their own notices; please review them before providing information.

10. Changes and questions

We may revise this policy and update its date. Material changes and new uses requiring consent will be handled in accordance with applicable law. Questions about this policy may be sent to [email protected]; this is the existing general administrative channel used to route a request, not a secure patient portal.

03Washington Consumer Health Data Privacy PolicyCategories, purposes, sources, sharing, and rights for applicable non-HIPAA consumer health data.+

Last updated September 8, 2026

1. Scope, categories, and purposes

This policy describes consumer health data, where covered by Washington's My Health My Data Act, received through this public website's related inquiry or privacy-request channels. LPMD Health LLC administers those channels for LPMDMG. It does not describe an entire treating program's clinical record system or information exempt from the Act, including qualifying protected health information governed by HIPAA.

The public site does not ask for health information or offer patient registration. A person may nevertheless include a health concern or care-related request in a message. In that circumstance, the categories received may include a described condition or symptom, requested or received care, a medication or treatment mentioned in the message, and identifying or contact information linked to that content. We process this information only to address the person's request, route them to the appropriate secure channel, handle a privacy request, or meet a legal duty, with the consent or other basis the Act requires.

The website does not use advertising trackers to infer health conditions, collect precise location to identify visits to healthcare facilities, or collect genetic or biometric data. Do not send sensitive health details in a general inquiry.

2. Sources of consumer health data

The source is the individual who contacts us, an authorized representative communicating for that person, or correspondence directly related to their request. We do not buy consumer health data for this public website, obtain it from data brokers, or build health profiles from advertising or browsing activity.

3. Categories shared and recipients

The information shared, when necessary and permitted, is limited to the identifying/contact information and relevant care-related content needed to address or route the request. Recipient categories are authorized administrative personnel, contracted communication or support processors, the appropriate clinical entity or care program involved in the request, and recipients required by law. Consent for sharing is obtained separately from consent for collection when the Act requires it. A general website visit is not that consent.

The specifically identified related entities to which information may be routed for the requested purpose are LPMD Health LLC and, as applicable, LPMD of CA PC, LPMD of NJ PC, LPMD of WY PC, LPMD of NE PC, LPMD of KS PC, and LPMD of TN PC. Listing these entities is not permission for unrestricted sharing. Information is not automatically distributed to all of them. An independent MSO or care program receives it only as needed for the person's request with the required authorization or other lawful basis.

We do not sell consumer health data received through these website-related channels or share it with advertising networks.

4. Exercising your rights

Where the Act applies, you may request confirmation that we collect, share, or sell your consumer health data; access to it and the required list of third parties and affiliates with whom it has been shared or sold; withdrawal of consent to collection or sharing; and deletion, subject to the Act's requirements and exceptions.

Submit a request to [email protected] with "Washington Consumer Health Data Request" in the subject, or through the contact form (opens in a new tab). Provide only the information needed to identify you and the request, and a safe way to respond. Do not include your medical history. We may verify identity or representative authority through an appropriate channel. We will act within the time required by the Act and notify you of a permitted extension.

If a request is denied, you may appeal through the same channel with "Washington Privacy Appeal." We will explain the appeal decision. You may also contact the Washington State Attorney General (opens in a new tab) with a complaint. Exercising these rights will not result in unlawful discrimination.

04Cookies & Privacy ChoicesWhat the page code uses, external font requests, and browser privacy signals.+

Last updated September 8, 2026

1. The page code

This public website does not set application cookies, use localStorage or sessionStorage, install advertising tags, or run behavioral analytics, retargeting, tracking pixels, or session replay. Navigation, expandable department descriptions, legal documents, and print controls work without storing a visitor profile. No optional tracking is activated by scrolling, opening a policy, or clicking a menu.

2. Hosting, security, and fonts

The absence of application cookies does not mean that no technical data is transmitted. Hosting and security infrastructure may keep request or security logs and may provide strictly necessary delivery or protective functions. Your browser may cache files for normal operation. Google Fonts supplies the typefaces and receives the technical data needed to deliver its stylesheet and font files.

Our external inquiry form is a separate service rather than an embedded form. Following a link opens that service, where its own cookies, storage, and notices may apply. The Website Privacy Policy explains these distinctions.

3. Browser choices and Global Privacy Control

You can manage cookies and other site data in your browser and enable supported privacy signals such as Global Privacy Control. This site does not enable sale, sharing for cross-context behavioral advertising, or targeted-advertising features, regardless of whether that signal is present. No extra "accept all" action is required to read this website.

Browser signal status is available when JavaScript is enabled. This page has no optional tracking to activate.

This page does not represent that a browser choice here changes your settings on a linked platform or deletes information previously provided in an inquiry. Use the request process in the Website Privacy Policy for information held outside your browser.

4. Changes to the site

Optional analytics, advertising, embedded forms, and other tracking features are not part of this site. If we introduce them, we will update the relevant notices and provide any legally required choices or consent controls before activating the affected processing. A cookie choice does not replace an authorization required for disclosure of protected medical information.

05Patient Privacy & RecordsHow to locate the treating entity, its privacy notice, and the correct records channel.+

Last updated September 8, 2026

1. Patient privacy is separate from website privacy

The medical entity providing your care is identified in your registration, consent, and care documents. Its applicable Notice of Privacy Practices and other patient notices explain how your medical information is used and disclosed and how to exercise your rights. This page helps you find that information; it is not itself a HIPAA Notice of Privacy Practices or a substitute for the treating entity's notice.

A shared brand, a common MSO, or a list of related professional entities does not establish a single covered entity or joint privacy notice. The privacy obligations applicable to a care program depend on its actual legal and operational arrangements.

2. Requesting records, a notice, or a correction

Use the secure patient portal or the records/privacy contact provided by the clinic or program treating you. Ask for the treating entity's current Notice of Privacy Practices, a copy of your records, a correction, a confidential communication arrangement, or other available rights. The responsible team will explain identity verification and the process that applies.

If you do not know which program or entity to contact, you may send a minimal routing request to [email protected] or the general contact form. Provide your name, the program name if known, and a safe callback method only. Do not attach records or describe symptoms. General administrative email is not a secure records-transfer method.

3. Respect for your information

Medical information is handled under the privacy rules that apply to the treating entity and the information involved. Administrative support does not grant unrestricted access to a clinical record. More protective laws may apply to certain records, including substance use disorder, mental health, reproductive health, HIV, genetic, or minor-consent information.

Where HIPAA applies, eligible patients may request restrictions on disclosure to a health plan for payment or healthcare operations when a service has been paid out of pocket in full, subject to legal requirements. Ask your treating entity how to exercise that right. A self-pay arrangement does not eliminate medical-record privacy responsibilities.

4. Concerns and complaints

You may raise a concern with the privacy contact for your treating entity. You may also use the general administrative channel above to ask for that contact, without including clinical details. Where applicable, you may file a privacy complaint with the U.S. Department of Health and Human Services Office for Civil Rights (opens in a new tab) or the relevant state authority. No one should face retaliation for raising a good-faith privacy concern.

06Cash-Pay Care & Good Faith EstimatesSelf-pay services, cost information, and federal billing protections.+

Last updated September 8, 2026

1. Our cash-pay model

LPMDMG's clinical services are 100% cash-pay and are not billed to insurance by the group. Each program explains its own fees and payment terms. A consultation fee may be separate from medications, laboratory testing, procedures, supplies, or another provider's services. Cash-pay care is not an insurance product, and reimbursement by an insurer or another party is not guaranteed.

Having insurance does not necessarily mean you must use it for a particular service, but government-program rules, benefit requirements, and provider arrangements can affect the available options. Confirm eligibility and payment responsibilities with the treating program before receiving care.

2. Your right to a Good Faith Estimate

People who do not have insurance or are not using insurance for their care generally have the right to receive a written Good Faith Estimate of expected charges for non-emergency healthcare items and services when care is scheduled in advance or when an estimate is requested. Ask the treating provider or facility for the estimate, including before you decide to schedule.

For care scheduled at least three business days in advance, the required estimate is provided within the applicable scheduling timeframe. You can also request an estimate without scheduling care. Ask each involved provider or facility about its expected charges; an estimate is not a bill or a promise that no unanticipated care will be needed.

3. When a bill is higher than expected

Keep a copy of your estimate. If a provider or facility bills you at least $400 more than its Good Faith Estimate, you may be eligible to use the federal patient-provider dispute resolution process. The process generally must be started within 120 calendar days of receiving the initial bill. Contact the treating program about a bill without delaying any applicable deadline.

Visit CMS medical-bill rights (opens in a new tab) or call 1-800-985-3059 for current federal information, eligibility requirements, and assistance.

4. What this website does not do

This informational site does not generate an individual estimate, collect a payment, enroll you in treatment, or determine the fee for your care. Request an estimate from the program and professional entity that will provide the service. A Good Faith Estimate of charges is different from a clinical eligibility evaluation, sometimes called a good faith exam.

07Accessibility & NondiscriminationAccess to website information, communication assistance, and reporting a barrier.+

Last updated September 8, 2026

1. Our commitment

Patients, prospective partners, clinicians, and other visitors should be able to access information and communicate with the medical group without unlawful discrimination. We aim to provide a usable public website and to address barriers brought to our attention. Care decisions remain based on clinical appropriateness, professional scope, service availability, and applicable law.

2. Website access

This site uses semantic headings, descriptive links, alternative text for meaningful images, keyboard-operable navigation and disclosures, visible focus indicators, and layouts that adapt to different screens. Reduced-motion preferences are respected. We use WCAG 2.2 Level AA as a design reference, not as a claim of an independent certification or a guarantee that every user, browser, or linked third-party service will have an identical experience.

3. Report a barrier or request assistance

For website assistance, email [email protected] with "LPMDMG Accessibility" in the subject. Describe the page or feature, the barrier, your preferred way to be contacted, and any useful browser or assistive-technology information. You do not need to provide a diagnosis or medical records.

For assistance with patient-care communication, disability accommodations, or language access, contact the treating program so the appropriate aid or service can be arranged under applicable requirements. General email is an administrative route, not an emergency or treatment channel.

4. External services

A linked form or outside website may have its own accessibility features and limitations. If an external form prevents you from contacting us, use the email alternative above. We will work with the relevant team to identify a usable way to provide the information or receive the request.

08Medical & Entity DisclosuresThe medical-group identity, named professional entities, and the MSO relationship.+

Last updated September 8, 2026

1. LPMD Medical Group

LPMD Medical Group, abbreviated LPMDMG, is the shared public-facing identity used to describe the medical group, its clinical services, and its clinical support organization. It is not a representation that all care is provided by one legal entity or that all entities listed below are responsible for every service.

2. The professional entities

The professional entities identified for the group are:

  • LPMD of CA PC
  • LPMD of NJ PC
  • LPMD of WY PC
  • LPMD of NE PC
  • LPMD of KS PC
  • LPMD of TN PC

The professional entity responsible for an engagement or encounter is identified in the relevant agreement, registration, consent, privacy notice, or other care documents. A state abbreviation in an entity name is not a public coverage map, and appearance in this list does not by itself establish authorization to practice in a particular state.

3. LPMD Health LLC and Exilara

LPMD Health LLC is the entity through which Exilara provides management services. Exilara is LPMDMG's primary servicing MSO. LPMD Health LLC is not presented as the treating medical entity and does not obtain authority to diagnose, prescribe, or determine clinical-quality findings by providing management services.

Aesthetic and Venture describe brands and engagement pathways within the Exilara portfolio. The actual contract identifies the responsible parties and services. LPMDMG also partners with independent MSOs; that relationship does not convert the independent MSO into a medical practice or transfer the medical group's clinical decision-making authority.

4. Clinical and operational authority

Appropriately licensed clinicians deliver care within their legal authority, education, training, and demonstrated competence. Medical leadership and Clinical Quality retain the clinical authority assigned to them. Clinical Operations supports workflow, systems, logistics, reporting, and coordination and does not independently direct diagnosis or prescribing.

An MSO relationship is organized around applicable state-specific corporate practice of medicine and professional-practice requirements. Neither this website nor the use of a particular entity name is a blanket certification of another organization's compliance. Engagement-specific ownership, control, contracts, licensing, supervision, and services require review.

5. Medical disclaimer and availability

No information on this site is an individualized clinical recommendation. Treatment and prescribing require an appropriate clinical evaluation and may not be recommended. Clinical programs, clinicians, visit formats, procedures, and medications are not universally available in every jurisdiction. Pharmacies, laboratories, and other third parties have their own professional responsibilities and terms.

For a medical emergency, call 911. For existing care, use the secure contact channel supplied by your treating program. Business inquiries are not a substitute for an appointment or clinical follow-up.
