Condition-focused programs

Start with the people who need care.

Care models developed around a defined condition or patient population, with the appropriate specialty expertise, clinical standards, and escalation pathways.

How the care is organized

Clinical responsibilities

A defined population

A focused program starts by identifying the condition or patient population it intends to serve. That definition guides the expertise, clinical standards, and follow-up the model requires.

The right expertise

Condition agnostic does not mean every specialty is available. Clinical services must match the qualified team’s actual authority, training, and demonstrated competence.

Clear escalation

A focused program needs to know the limits of its scope. Referral and escalation pathways help the team respond when a patient needs information, expertise, or care beyond the service.

Questions about the group

Common questions

Can an MSO propose a new care program?

Yes. LPMDMG welcomes conversations about new care models. The patient population, expertise, states, setting, and delivery requirements are reviewed for each engagement.

Does the group support men’s and women’s health programs?

Defined men’s and women’s health services are among the focused program models considered, subject to clinical scope review. This is not a claim that every service is currently available.
